Living Well Disability Services delivers exceptional services that transform the lives of people impacted by disabilities. With 36 group homes throughout the 10-county area we specialize in providing a holistic approach to wellness and well-being for the people living in our homes.

As a Staff Nurse at Living Well Disability Services you will provide direct nursing support and oversight of medical supports to people living in our homes. This includes partnering with the home manager and staff to ensure cohesive medical supports are provided according to the needs of each person, providing direction and supervision of unlicensed staff regarding health-related issues within the scope of nursing practice and working directly and effectively with people receiving services, staff and others as needed to provide optimal health care to each person. Each nurse has a caseload of approximately 1-4 homes. Key areas of accountability include (but are not limited to):

  • Planning: Follow medication and health care policies and procedures as determined by the organization. Perform ongoing evaluation of each person’s status to assure appropriate health care is being provided. Assist in the development of health care profiles and nursing/health care assessments as needed.
  • Organization: Effectively train and mentor staff to develop the skills necessary to assist and support people to achieve desired health care outcomes including the use of medications and treatments. Ensure staff understand the specifics of each person supported’s health care plan and needs, providing additional training as needed.
  • Communication: Communicate effectively and cooperatively with a variety of people including providers, interdisciplinary teams, and organizational teams. Ability to answer health and medication questions about people supported.
  • Teamwork: Work as a team members to support all people to help them maintain the health they need to access their communities, act on potential, and meet their desired outcomes.

Qualifications:

Must have current LPN license, MN Drivers License, and ability to pass a MN DHS Background Study.
